Driving Accuracy, Accountability, and Community Service Through Rigorous Quality Assurance.

The Quality Assurance and Capacity Building Case Team has officially launched its Q2 EQA Operational Plan, setting in motion a focused, two-month initiative designed to dramatically enhance diagnostic reliability for Tuberculosis (TB) and Malaria across our facilities. This isn't just a technical exercise; it is a life-saving imperative—every slide re-read, every data point tracked, and every standard enforced directly translates into faster, more accurate patient care and stronger community health.

The Shift to Proactive Quality Management

To move beyond reactive fixes, we have established a new system of zonal accountability. Every assigned regional staff member is now mandated to treat their facilities as their primary responsibility, ensuring tangible quality outputs daily.

Mandate: The Weekly Performance Report

The team is required to submit a comprehensive Weekly Facilities Performance Report, focusing not just on specimen data (positives/negatives) but critically on the Gaps Identified and the status of Corrective Actions given the previous week. This ensures every identified issue reaches closure and drives continuous improvement.

Meeting National Indicators: Our Life-Saving Scorecard

Achieving National Standards requires vigilance across all phases of the lab service. Our team is now monitoring the following Facility-Level Quality Management System (LQMS) Indicators, where failure to meet the target can have dire consequences for patient safety and resource utilization:

Pre-Analytic Phase (Safety & Resources)

  • Specimens Rejection Rate: < 2% (Protects patient safety and resources.)
  • Supplies Stock Outs Rate: < 5% (Ensures service continuity.)

Analytic Phase (Technical Competence)

  • EQA/PT Performance: > 80% (Validates reliable results.)
  • IQC Coverage: > 90% (Ensures daily analytical stability.)
  • Equipment Downtime: < 5% (Measures operational efficiency.)

Post-Analytic Phase (Service Impact & Trust)

  • Turnaround Times (TAT): > 80% (Enables prompt clinical action.)
  • Customer Satisfaction: > 85% (Fosters clinician trust.)
  • Standard Test Menu Coverage: > 95% (Ensures comprehensive, local care.)

Addressing Frontline Challenges: A Team Commitment

During the launch meeting, the team—including Coordinator Ahmed Suane and Officers Abdi Haybe, Nimcan Abdisalam, and Taleex Abdi—confronted serious logistical obstacles head-on.

Core Problems Identified:

  • Slide Integrity: Insufficient slide volume and critical issues with broken slides, dirty slides, and lack of orderly storage.
  • Data Management: Inadequate recording and registration practices.

Team Resolution:

The team unanimously promised to work closely, ensuring that every staff member assigned to a specific zone immediately implements targeted corrective actions to safeguard the quality and integrity of the EQA samples. This commitment to overcoming these physical challenges is our highest priority.

The Q2 cycle is more than an audit; it's an intervention. By adhering strictly to National Standards, driving high LQMS scores, and maintaining rigorous accountability through weekly reporting, we are actively strengthening our diagnostic foundation. We are building systems that save lives.

The Supplies Stock Outs Rate (< 5%) indicator ensures service continuity. Keeping this rate low confirms that reagents and essential kits are always available, which is fundamental to maintaining a functional lab and avoiding the necessity of referring patients elsewhere.

Analytic Phase: Ensuring Technical Competence

EQA/PT Performance (> 80%) is the ultimate validation of technical ability. Consistently scoring above 80% in external quality assessment schemes demonstrates that your laboratory is producing accurate, reliable results that are comparable to high-performing facilities worldwide. This is vital for building internal and external confidence.

IQC Coverage (> 90%) ensures that every day, tests are performed under controlled conditions. High IQC coverage means the analytical system is stable and results are trustworthy before they are released.

Equipment Downtime (< 5%) measures the efficiency of maintenance. Unscheduled downtime means cancelled tests and delayed results. A low downtime rate signifies successful preventive maintenance and efficient management, ensuring consistent operational capacity.

Post-Analytic Phase and Management: Measuring Service Impact and Trust

Turnaround Times (TAT) for Test Results (> 80%) is arguably the most visible sign of a lab's efficiency. Meeting this target means your results are consistently delivered when clinicians need them, enabling prompt and correct clinical action.

A high Level of Customer Satisfaction (> 85%) confirms that clinicians and patients are receiving the expected level of service, fostering stronger collaborative relationships within the health facility.

Monitoring Test statistics (Reported Monthly) is essential for workload planning, resource allocation, and identifying trends in disease prevalence, which informs public health policy.

The Proportion of tests performed against minimum standard Test Menu (> 95%) ensures the laboratory meets the basic diagnostic needs required for its level of service, preventing unnecessary referrals and ensuring comprehensive care is locally accessible.

We are proud to announce the successful completion of a structured technical support mission at Qabri-Bayax Primary Hospital, conducted from October 9-11, 2025, by the Somali Regional Health Bureau’s Laboratory Directorate. The core objective was to strengthen laboratory service quality, documentation systems, and operational readiness, marking a significant stride toward laboratory accreditation and improved patient care.

Key Achievements and Deliverables

The mission, led by our Quality Case Team, was a highly collaborative effort, with the hospital's Quality Improvement Team and Laboratory Head working side-by-side with the visiting experts. Key achievements that will sustain this new focus on quality include:

  • Establishing a Quality Baseline: A comprehensive baseline assessment was completed using the SLIPTA Checklist (Version 3), providing a clear picture of the laboratory’s current status and pathway for improvement.
  • Implementing International Standards: New ISO 2022 laboratory documents were customized to fit the local context, then printed, labeled, and organized into a dedicated, accessible shelf.
  • Empowering the Staff: The hospital staff received hands-on orientation on how to use the new Standard Operating Procedures (SOPs) and formats, ensuring that quality practices are not just written—but integrated into daily work.
  • Strengthening Management Systems: A systematic document control system was setup, and a complete inventory of all lab equipment was carried out and recorded.
  • Committing to the Future: Hospital leadership demonstrated their dedication by committing to prepare a dedicated office space for the Laboratory Head and Quality Team to ensure this momentum is sustained.
